Ending PF and ESI Coverage: What Documents Do I Need to Finalize This Process?

Dear Seniors, I am working in a transport company. I would like to end the PF and ESI coverage of our company. Could you please inform me about the documents needed at the time of final coverage? The company has not yet finalized the coverage. Thank you.

Required Documents for ESIC & PF Coverage

The following documents are required for coverage under ESIC & PF:

1. Form 5A in duplicate
2. Proforma for coverage in duplicate
3. Photocopy of Memorandum and Articles of Association
4. Certificate of Incorporation
5. Copy of PAN card
6. Particulars of Owners/Directors (Annex-I)
7. Details of employees in duplicate (Annex-II)
8. Employee and Employer contribution (Annex-III)
9. A demand draft for Rs_____/-
10. Authorized signatory letter
11. Balance sheet
12. First sale bill
13. Factory Licence
