How Should We Handle a Second Probation Extension Due to Performance Issues?

We have one employee who joined us on 01.07.2011, and his confirmation is due on 01.01.2012. However, due to performance issues, his probation got extended for 3 months, i.e., until 31.03.2012. Even after that, the boss would like to extend it for another 3 months.

I am requesting a format for the same since this kind of issue with us is the first time. Please do the needful.

Simply, you can provide a letter stating that due to performance reasons, the probation period shall be extended until a specific period.

1) What are the reasons for extending the probation period consecutively?
2) Is it really necessary to do so?
3) Does his superior have any personal bias against the probationer?
4) Will this extension affect other probationers as well?
