Please read CB Gupta's "Human Resource Management." There are various methods in this book to conduct performance appraisals, including a 360-degree look. You can also refer to any Symbiosis MBA HR book for more information. The book covers both traditional and modern methods of performance management with a very important one being MBO. I am familiar with a simple ranking method that I have been appraised with twice.
