How Do I Write a Reference Check Letter to a Previous Employer? Need Formats and Tips

Please help me with the reference checks. How do I send a letter to the previous employer for the verification of the employee? Please assist me with the checklist or formats.

Peer has done a good job for you. Thanks, Peer. Earlier, 1-2 times I have uploaded the Reference Check pro forma. Please check on this site or the links that Mr. Peer has provided. Many times, companies do not respond to the Reference Check questionnaire. Finally, I started doing the Ref Check on the telephone itself. I used to make a note of the person with whom I spoke, his/her mobile number, designation, etc. You may use the same formula.
