How Can I Request a Job Transfer Closer to Home? Seeking Advice and Sample Letter

Please could someone help me? I need a letter asking my manager for a transfer to one of our other day homes. I work with clients who have challenging behaviors. Where I am based is so far from where I live, and they have homes for our clients within my area where I live. I don't know what to say that would help me get a transfer near to where I live. Please help me.

I really love my job, but it is based so far from home. If anyone could please send me an example letter, it would mean so much to me. Much appreciated.

As I cannot do as many shifts as I would like to, the driving back and forth is really too much for me.
