Hi Prasanth,

I have a lot of information about Root Cause Analysis for the Service Industry. Earlier, I had posted service industry RCA details on this blog in 2007; unfortunately, someone deleted it. Anyhow, I am sending customized root cause analysis categories.

We usually do RCA with categories of man, material, machines, viz 5M's. This may not be suitable for all industries. For your service industry, as you are not dealing with any machines or materials, you can customize in the following way:

- Lack of standards - work instructions
- Lack of procedures
- Lack of training skills
- Lack of competencies
- Lack of specified formats/templates/documents
- Lack of processes - sequential operations
- Lack of systems - relevant IT tools/packages
- Resource constraints
- No checkpoints - milestones/major operation reviews, etc.
