Form 5 And 10 Of PF

Dear All,
Please let me know from which source the inputs or information for filling up form 5 ,10, amd annual returns 3a and 6a will be done.

Form 5 in PF Return should be prepared by entering the Newly joined employees which will be available (Name, Father's Name, Sex, Age/Date of Birth, Date of Joining) in your Employees Register or Register of Workmen. Form X in PF Return prepared with the details duly available in the same Employees Register or Register of Workmen those who left the organisation. With the help of monthly PF Remittance details, you can simply compile the data for preparation of Annual Return.
