The employment form consists of the following details (in order):
1) Reference (either through any consultancy or any employee reference)
2) Position applied for
3) Name
4) Local Address / Permanent Address
5) Marital Status (Married/Unmarried)
6) Name of the spouse
7) Educational Qualification
8) Professional Qualification
9) Name of the previous company, Designation, Job Descriptions, Starting salary, Salary when left
10) Salary break-up in your current organization
11) Major tasks and job responsibilities
12) Strengths & weaknesses
13) Any criminal records
14) Operations or medical reports
15) References from the previous employer
16) Remarks to be filled by the panel member who conducts the interview (whether the candidate has been selected or rejected, salary negotiated, post for which selected, and signature of the person filling in the remarks).
