Need Help with Joining Report and Employee Personal Data Form for New Hires?

Can anybody send me a format of a Joining Report and Employee Personal Data Form that is supposed to be filled when a new employee joins an organization?

Hi Rivti,

In case you still haven't received any answers on the employment form, let me give you a brief outline of it. Since I cannot provide you with a document, I think the following structure/details would be beneficial and helpful to you.

The employment form consists of the following details (in order):

1) Reference (either through any consultancy or any employee reference)

2) Position applied for

3) Name

4) Local Address / Permanent Address

5) Marital Status (Married/Unmarried)

6) Name of the spouse

7) Educational Qualification

8) Professional Qualification

9) Name of the previous company, Designation, Job Descriptions, Starting salary, Salary when left

10) Salary break-up in your current organization

11) Major tasks and job responsibilities

12) Strengths & weaknesses

13) Any criminal records

14) Operations or medical reports

15) References from the previous employer

16) Remarks to be filled by the panel member who conducts the interview (whether the candidate has been selected or rejected, salary negotiated, post for which selected, and signature of the person filling in the remarks).

Hi Rivti,

When you mentioned that you wanted a joining report, I immediately thought of the new joining report that is used for management. Because when we say 'joining report', it should ideally include the name of the new joiner, date of joining, department joined, number of years of experience, employee ID, etc.

Anyways, the one mentioned may also serve some purpose.

Yes, as Deepa said, we generally maintain a joining report to track all candidates who are yet to join. The above data can also be used, but it's primarily used once after a candidate joins. Attached file can be used as a template for the joining report.
