How Do I Write an F&F Letter to Request My Remaining Salary from My Company?

I wanted the format of an F&F letter asking for the remaining salary from my company.

Please find attached the full and final documents of our organization 'Global Overseas'. Our organization provides all benefits to employees who are leaving, including one airfare to a managing level employee (to any destination in India with one family member).
