I have to prepare a quarterly performance review format. Depending on which our employees will be getting incentives as it is a web development company. As their level will be decided every quarter end. Please go through the format I prepared and give me suggestions if any modification required.
What I have done so far is:
1) Discussed the KRAs with the senior person of a particular position on which the performance will be reviewed.
2) Now I want to put some lines that can judge their coordination within the team and their overall behavior (As it's very common here that I get complaints about team members by each other). Please suggest some more points if I can add.
