The risk assessment sheet which you shared containing bare minimum minimum things only... Also it's lacking responsibility and accontability column... Untill ands unless it's not there, this sheet of no use... nobody is not going to implement anythjing...

Thank you Mr. Dipil for your comments. I made this on behalf of our safety person. I am basically from IR department and I made this only by viewing the acts happening in Site. I feel it may be useful to share with my counterparts.
