I am not very sure what you are looking for. Are you trying to evaluate training? In that case, you should probably have a look at Kirkpatrick's model.

Thank you for your reply. Actually, I need to assess the training effectiveness of all the training sessions conducted in a specific year. I am planning to conduct an effectiveness survey using a questionnaire. Could you please assist me in determining the questions that should be included in the questionnaire?

Your questionnaire will be based on the training you have done in your organization. We do not work for your organization, so we cannot devise the questions for you. If you are looking at the effectiveness of training overall, then you need to talk to the various divisions in your organization and determine if the training has changed anything. Is the company more profitable from training staff to do jobs better or more efficiently? Has training improved the overall loss of downtime due to accidents? Is there better morale and teamwork, etc.? Only you can devise the questions that need to be asked based on your particular organization.
