Understanding Control Groups in Kirkpatrick's Model: How Do They Impact Level 3 Analysis?

Hi, I have been working on Kirkpatrick's model for quite some time now. I have come across some very interesting articles through all the links posted here. I am trying to understand the importance of using a Control Group for the analysis of Level 3 effectiveness of the model.

Also, could anyone help me with a questionnaire for level 3-4 analysis?

For Kalpana: The questionnaire that you have given is not suitable to measure the Level II change. To measure the Level II change, you must conduct the post-training test. Merely asking the participants whether they have implemented the learning or not is insufficient. It is understood that nobody will say 'no'. Level III change is measured through changes in various ratios. Level IV change is measured by evaluating the business impact resulting from these changes in ratios.

Actually, this is the questionnaire for Level III, not Level II. Level II was completed through pre-test and post-test analysis. I had only provided the questions. I am unable to locate the exact questionnaire at the moment. Once I find it, I will also post that. 
