Required Details On Conducting Performance Appraisal Policy

Dear all,
This is Rekha Rawat I am a HR in one of the small IT company, Noida.
It seems to be very difficult for me to initiate a process for performance appraisal.
Can someone help me out in knowing the process of conducting a transparent appraisal system which could be implemented in a low or mid level company.
and what is the rate of increment that can be given and what is its procedure.

Dear Rekha,
The following is the process which we have for our company for Performance Appraisal...
1. Define the guidelines and the time frame of each process in Performance Appraisal
2. Issue the performance appraisal forms to all the units / offices / branches...
3. Inform all emloyees to fill the self appraisal part within the time frame set by the HR department
4. The forms will then go to the respective HODs for the evaluation and the ratings will be given by them...
5. THe forms will then come to the HR department within a speculative time frame...
6. The HR Dept. will then work on an excel sheet and the data will go for the approval from MD / VP - HR....
7. THe HR Dept will then issue the increment letters and / or promotion letters to the employees....
