Struggling to Find Form 5A for National Holiday Work Approval – Can Anyone Help?

I need Form 5A to obtain approval from the Inspector of Factories to work on NATIONAL HOLIDAYS. I could not find this form on the internet. Could someone kindly provide me with the FORMAT for FORM 5A?

Dear Member, Various forms are provided by the Factory Rules (Central & State). Therefore, first decide which of the State or Central Factory Rules are applicable in your case, and then go through those Rules to obtain Form 5A if you are unable to find it online.

Availability of Form 5A

The Form 5A is available in the Factories Act, 1948, and the procedure is also mentioned in the Act.

I agree with Mr. Khola that you must specify in which state your factory is operating. Additionally, I would like to inform you that you can obtain Form 5A from the website of the Ministry of Labour and Employment. The link is labour.nic.in/.
