Dear Anupama,
Do you have so many disciplinary cases pending in your company that it has led you to consider forming a disciplinary committee? Please create a separate chapter in your HR Manual for Discipline. In this chapter, outline which cases are considered disciplinary, those that warrant disciplinary or administrative action.
Define the types of punishments that can be awarded (Minor Punishment / Major Punishment). Nominate a Presiding Officer and two members for the committee. The Presiding Officer should be at the GM level or an individual authorized to conduct proceedings, with one member being their superior officer and the other an HR representative.
You may convene as a Board of Inquest and, as appropriate, impose sanctions that will be factored into the employees' appraisals. If you have numerous ideas in mind, carefully develop and seek approval for your proposals from the appropriate authority. Add these guidelines to your HR Manual, either as an addendum or an annexure, or as a new chapter.
