It is observed that State & Central Rules have different sets of annual return forms while you have not mentioned the name of the Maternity Benefit Rules under which you are requiring these formats.

Then in that case you are required to submit annual return in form K,L,M & N as per Rule 16 of the Haryana Maternity Rules, 1967. Forms are attached herewith for your ready reference.
