I think you need to change the scale for measuring performance because you have mentioned both types of questions (positive and negative). This makes it difficult to evaluate in X & Y.

Yes, Faruk, you are right. However, I feel we cannot keep only positive or only negative feedback for the appraisal process. It is essential to consider both sides. Can we assign weights to different aspects? Also, should the format be in MS Word?
