How Do You Use Promotion Recommendation Forms to Support Employee Growth?

The attachment is the form we use during the year in case the Line Manager wishes to recommend his/her employee for a promotion. Hope you find it useful.

Hi there, I have a query: why do you have a separate form to recommend employees for promotion? To the best of my knowledge, promotion recommendations are included in the annual performance appraisal form. Don't you think you are hindering the process by having a separate form for it? Not to mention, you're wasting paper.
