Can somone share a format for an agreement to be used for associations with clients for providing HR consultancy services (not for executive search /recruitment ).

I believe you should do some research and prepare your self the agreement. Format you can get from google too.
Its upto you how you long the terms and condition you want.
By your post I am sure you are looking to enter in HR solutions and Services . So being a competitor I wont be able to provide you the same , unless know the purpose
