We have recently submitted ESIC half yearly return of contributions to local branch office.
We have only 30 employees which are covered under ESIC hence we do not require "Certificate by CA".
But ESIC office has stamped "SELF CERTIFICATE NOT APPENDED" on our acknowledgement copy of the return.
We are submitting the R.C. in the revised format as per notification issued dtd 14th March 2008.
Please help what is the format of "SELF CERTIFICATE" which we need to append.
There is no format on website of ESIC where we prepare online return of contributions.
