Can anybody tell me how to prepare a Termination Letter for an employee's services or a Notice of Termination from the job? I urgently need this information.

Sample Termination Letter For Firing a Bad Employee
Mr. Thomas A. Lasker c/o Blank Company Inc. 76554 South Main St. Anywhere, AnyState 99999
Dear Mr. T. A. Lasker,
Your employment with Blank Company Inc. is now terminated. Recent events and work-related issues, such as miscalculations on your paperwork and missing company funds, have led us to this decision. Despite previous warnings, you have failed to provide satisfactory explanations for these errors. Therefore, we believe that you are not meeting our job performance standards.
Blank Company Inc. would like to offer the following terms to assist you in transitioning to new employment and acknowledge your years of service with us:
1) Blank Company Inc. will continue paying you 100% of your salary for the next three months. If you secure alternative employment within this period, Blank Company Inc. will provide a lump sum equivalent to 35% of the remaining wages.
2) All accrued vacation days and sick leave will be paid in full within the next three months.
3) Your benefits, including health and dental coverage, will be maintained for six months, unless acquired by a new employer during this period.
4) We have arranged for an employment counselor to assist you in finding future job opportunities.
These terms are subject to the following conditions:
1) You must maintain the confidentiality of any proprietary information obtained during your employment.
2) You are required to inform Blank Company Inc. of any new employment within three days for the next three months and update us on any changes in benefits for the following six months.
3) A signed copy of this letter must be returned to Blank Company Inc. within five days of the date above.
We would like to schedule a meeting with you to facilitate the retrieval of your personal belongings from the office. Please return any keys, documents, or company property still in your possession.
