Don't waste your time making appointment letters for drivers. As per the Karnataka Shops and Establishment Rules, we are supposed to issue Form 10. This Form 10 serves as the appointment order by the employer. I usually issue Form 10 for lower-level positions and also attach standard terms and conditions of employment.

You can find the required form in the Shops and Establishment Act or the factory regulations of your state.
