Dear Friend,

You can use a very general format for attendance wherein you can put the following headers:
Name Department Time in Sign Time out Sign

You can also add a movement column to track the movement of employees throughout the day with the following format:
Name Time out Sign Time In Sign Purpose

The terms are explained below:

- OH = ??
- OD = ??
- CA = ??
- CL = Casual Leave
- CO = Carried Over
- EL = Earned Leave
- HL = (in lieu of work on) Holiday Leave
- LWP = Leave Without Pay
- SL = Sick Leave
- Cr. B/F = Credit Brought Forward
- Cr. C/O = Credit Carried Over

Note: Credit B/F and Credit Carried Over are used when the salary payable after deductions is rounded off to the nearest Rupee or nearest Ten Rupees. The amount excess (-)/short paid (+) is carried over and the same is brought forward next month for adjustment.

Attendance sheet must contain:
- Total working days
- Total weekly offs
- Total holidays
- Total leave adjustments (CL, SL, PL)
- Total absenteeism (LWP)
- Total pay days

For the time recording sheet:
- Must have card punching by employee or signatures on both at the time in and out.

Kindly prepare an attendance cum salary sheet, which can provide a record of entry, tea time, lunch break, and exit of the employees. This should also include the total working hours per day and the calculated salary per employee on a monthly basis based on the working hours.
