Dear Swaminathan,
Due to the liberalization of medical facilities for the insured person, the insured person can now request the concerned hospital authority to refer his case to any of the ESI tie-up hospitals throughout India, as per his preference, in cases of serious injuries.
For routine medical facilities, as the employee was transferred to Chennai, please ask him to submit ESIC-53 form at either the old or new ESI branch office to transfer his dispensary from Chennai to Madurai.
Regarding medical facilities for his family, if he is working away from his family, he can request the concerned ESI branch office to provide a separate ESI card for family use, which would be beneficial for their family in Chennai.
Additionally, if he is enrolled under Project Pehachan and has enough points, he will be provided with two smart cards – one for himself and another for his family members. These cards can be used at any dispensary or hospital across India after the full computerization of all its dispensaries and hospitals. This process is estimated to take another 2-3 months.
I hope I have clarified your doubts to some extent.
All the best.
