Contractor Labour Lincence

can anybody help me, what r the documentations is required for a labour lincence for a contractor & also need to be submited to get a lincence.

1. form IV application in triplicate
2. form V from prinicipal employer.
3. work order copy.
4. security Deposit amount and license fee

good information....
but in Provident fund and Misc act 1952, the description of form 10 D is incorrect as it is used in case of death of member instead of form 20
Pls correct me if wrong

Dear Sir,
On behalf of Work Order/ Job contract you may issue Form V from your side (if your are principle employer). On behalf of Form V Labour Office will provide you a challan for deposit amount on the basis of no. of job hands. A copy of challan vide which you have deposited licence fee, may be submitted until you get Labour Licence from the Labour Office.

Following documents are required for obtained CLA license from the Labour office,
1. form IV application in triplicate
2. Annexure B along with form IV
3. form V from prinicipal employer.
4. work order copy.
5. Affidavit
5. security Deposit amount and license fee

For contract labour licence under Contract Labour Act 1970, form II also should be attached.
For licence under Inter-state migrant workmen Act 1979 ,the following documents shall be submitted:
Form II
Form V
Form VI
Work order copy - not a must.
Challan copies for payment of security deposit and licence fee
