Why don't you convert it into Excel? It will be more helpful for you. Are the criteria meeting all your needs?

Why do you need age when you already have an option for date of birth, and converting it into Excel is a better option.

Hi there, why do you need to put age when you already have the DOB? Also, why do you also require a person's weight? Asking for height is sometimes okay as it can be helpful, and it is also good to ask for the number of dependents rather than an exact bifurcation into male/female. You can judge whether that fulfills your requirement or your organization's.
