why do u need age when you already have an option for date of birth and converting it into excel is a better option. Regards Ankit

Hi there, why do you need to put age when you already have the DOB? Also, why do you also require a person's weight? Asking for height is sometimes okay as it can be helpful, and it is also good to ask for the number of dependents rather than an exact bifurcation into male/female. You can judge whether that fulfills your requirement or your organization's.
