This is to inform you that if you wish to apply for the ESI subcode, please ensure you have already filled out Form C-11 and completed Form No. 01. Once done, attach Form C-11 and submit it to the ESI office along with a covering letter.

Obtaining an ESIC Sub-Code

C-11 is a certificate issued by the department as an ESI main code. To obtain a sub-code, a copy of C-11, the branch office address, details of employees, MOA, and AOA of your organization are required. The list of directors, all duly signed and stamped, has to be submitted to the department of the desired area to obtain the sub-code.
