Need Help Updating Our Performance Appraisal Format for a Small IT Company – Any Suggestions?

This is Pranavi. I am working as an HR executive in a small IT-based company with 95 employees. Can anyone help me with modifying our performance appraisal format? Please suggest some appraisal formats.
