Unemployment Certificate Requirement for PF Withdrawal

Is it necessary to provide an unemployment certificate along with Form 19 and 10C in case of withdrawal of PF from a previous organization? If yes, please provide me the required format of the same.

I resigned from my job in October 2012 and paid off one month's salary. I would like to withdraw my PF. Could you kindly suggest the procedure? My question is, since it has been four years, will there be any issues with withdrawing PF?
