Just write a letter to the Bank Manager stating that you would like to close your current account and mention the reason for closure. That's all.

Telephone your bank and ask! Maybe you need to take a little time to think about things before you ask questions like that.

There is no standard format. The normal practice is that you need to make an application to the Branch Manager of the bank on your letterhead and get it signed by all the authorized signatories (this is in case the current account is not that of a proprietorship firm). You will also have to surrender unused cheque leaves.
