Dear Shakti, In Form 19, you need not enter the contribution particulars. The particulars will be available in Form 3A submitted by your ex-company in March 2009. If the actual relieving was in March 2009 (meaning the currency month of April 2009, pertaining to the 2009-10 year), then that month's contribution will be available in the annual returns of 2009-10. If not, the PF Office will request the same from the employer.
You can provide the old address because the PF code might remain the same.
Form 10C: Scheme Certificate or Withdrawal
In Form 10C, the question of whether you would like to accept a scheme certificate should be answered based on your preference regarding whether you want the withdrawal benefit of the Pension Fund or if you would like to retain it to receive a scheme certificate from the Employees' Provident Fund Organization. This certificate will verify your service, which will then be added to your service with any new company. By accepting the scheme certificate, your previous service will be retained. When you join another establishment where EPF is applicable, you can surrender the certificate, and your past service will be added to your upcoming service. The pension amount will be determined considering the total service. To have your past service added to your new service, you must opt for the scheme certificate. In that case, you need to enclose the birth certificate of your children along with the forms. Alternatively, if you do not wish to obtain any certificate but prefer to withdraw the pension fund amount, you can select "NO."
Please include a cancelled cheque leaf to facilitate NEFT by the EPFO.
