In form 19 you need not enter the contribution particulars. The particulars will be available in the form 3A submitted by your ex-company in March 2009. Or if the actual reliving was in the month of March 2009 (which means currency month of April 2009 which pertains to 2009- 10 year) then that month's contribution will be available in the annual returns of 2009-10. If not, the PF Office will call for the same from the employer.
You can write the old address because the PF code might remain the same.
In form 10C, the question whether you would like to accept scheme certificate is to be answered with reference to your choice as to whether you would like to have withdrawal benefit of the Pension Fund or would you like to keep it as it is and get a scheme certificate from the Employees Provident Fund Organisation which will certify your service which in turn will be added to your service with any new company. By accepting the scheme certificate your past service will be retained and when you join another establishment wherein EPF is applicable, you can surrender the certificate and by doing so the past service will be added to your forthcoming service. Pension amount will be decided taking into account the total service. If you wish to get your past service added to your new service you have to opt for scheme certificate. If so, you have to enclose the birth certificate of your children also along with the forms. On the otherhand, if you do not wish to get any certificate but would like to take back the pension fund amount also, you can tick "NO".
Please attach a cancelled cheque leaf for effecting NEFT by the EPFO.
