Before thinking of any such letter you should explore all the possibilities on the ground of reality & truth. Judgement should not be biased. Check with other employees, check the work front, and reason behind uncooperation. With patience find the root cause, investigate and then search for other solution.

Please do specify the level or the kind of act which he/she had performed for us to provide a proper solution..
