What is the procedure to take duplicate PAN Card?

dear members,
My colleague had missed her Income Tax Pan Card.
She want duplicate PAN Card.
what is the procedure to take duplicate Card?
Please tell me.

First lodge an FIR with your area police station reporting loss of PAN card.
Then download the form for issuing duplicate PAN card from the website of the Income Tax Department itax and submit it along with a copy of the FIR to the nearest office/ processing centre of UTISL or NSDL.
The list of city wise processing centres is also available on the website.
