Hi Geetika,

This is Vijaya. Your form is a good beginning. You may also include the following:

1. The introduction to people session
2. The specific requirements of work tools/equipment needed
3. Some feedback on the company's goals/departmental objectives

These additions will help us to assess the actual effect of any induction program.

Hi Geetika,

This has come up well. For further inputs, you may refer to Q12 from Gallup. It would be worthwhile if you extend this process to 30-day and 90-day feedback as well (wherein 90-day feedback is from the superior about how the candidate has settled in his/her job).

I would suggest if we could have an onboarding feedback form and an induction feedback form separately.
