This is vijaya. Your form is good beginning. You may also include the following:
1. The intro to people session
2. The specific requirement of work tools/ equipments needed
3. Some feedback of company's goals / departmental objectives
These will help us to judge the actual effect of any induction prgram

Hi Geetika,
This has come up well. For further inputs you may refer to Q12 from Gallup.
It would be worthwhile, if you extend this process to 30 day and 90 days feedback as well (wherein 90 days feedback if from superior about how the candidate has settled in his/her job).
