Confused About Form-V for Worker Registration in Haryana? Need Help with Format and Details

Dear Friend,

Our company is a construction company. Our principal employer received a notice from the Labour Department of Haryana to submit the "Record of worker beneficiaries/registration in Form-V of all workers on-site working with each contract." Can anyone provide the format or details to be included in the form? Can anyone clarify whether it is a form or a register?

Please assist with this matter.

Dear Shiv,

This is to inform you that the Labour Department (Factory wing) has issued a notice for compliance under The Building & Other Construction Workers Act, 1966 and The Haryana BOCW Rules, 2005, regarding the submission of Form V (Registration form) and the record of beneficiaries. The record of beneficiaries is a document containing details of registered beneficiaries, and Form V is an application for registration to access benefits provided by the Haryana BOCW Board.

Both required formats are attached herewith for your reference.
