Understanding the Labor Authority's Full and Final Settlement Format: Can Anyone Help?

Can you please let me know the format of the full final settlement prescribed by the Labor Authority?

Hi,

Full and final settlement includes his final settlements, his EL encashment, last salaries, any extra amount payable to him in the form of incentives/bonus, arrears if any, and gratuity.

Dear Navin Agarwal,

This is to inform you that as far as I know, the labor authorities have not prescribed any format for making the full and final payment to the employee. Labor laws have also not provided us with this type of format. Moreover, you have also not mentioned the name of the central or state labor authority.
