I would like to write a letter to the employee who has been breaking the rules (smoking in the non-smoking area). Can anyone please help me by sending a sample.

Warning Letter

You were on duty in the shift on --/--/09. During the same shift, at about --:-- PM, Mr. ABC of the ABC Dept, who was on his rounds, found you smoking inside the toilet washing area. This incident was reported to Mr. DEF, who was the shop floor supervisor during the shift. When ABC and Mr. DEF asked for an explanation from you regarding the incident, you expressed regret and assured them that you would not repeat it in the future.

Your act of smoking inside the factory premises is prohibited since our facility has been designated as a completely Non-Smoking Facility since --- 2009, and all employees were informed about this through our notice dated ---.

Here is what I would do. I would have the security guard write him a warning "challan" a couple of times. The third time, classify him as a repeat offender and impose a fine. The fourth time, impose a heftier fine along with a written warning for suspension. The fifth time, follow through with your earlier warning (suspend him from the job without pay). That would get his attention! :-)
