I am seeking guidance to evaluate an employee's performance based on an appraisal form. I have framed this appraisal form for my organization, in which there is a provision to mark an employee's behavior on the rating scale from 1 to 5. There are also columns where employees shall write what they feel about their achievements in relation to the targets set initially. Please help me with the procedure of evaluating such a form.
