Struggling to Write Appraisal Letters? Seeking the Best Formats for Designation and Salary

Hi everybody,

Can anybody help me with this matter? I am working as an HR in an IT company, and I have to provide my employees with appraisal letters. However, I am struggling to find a good one. Could somebody please share with me the best appraisal letter format for both designation and salary appraisal?
