Dear Samir,

Who will help you when one does not know about your appropriate Government? Whether this is Central or State, and if State, which is the State? Each and every state has its own rules. Different states may have different formats for maintaining the registers. First, decide this issue and then appeal to the concerned HR members for the purpose.
