P.F. Transfer - Provident Fund

Hi,
One of our employee was transfered from Mumbai Branch to our Coal Mines located at Madhya Pradesh Br. where the Coal Mines Provident Fund applicable.
Please tell me how to transfer his provident fund balalnce from Employees' Provident Fund Scheme to Coal Mines Provident Fund.

Hi Vaibhav,
The employee has to fill Form 13 and send to PF Commissioner to transfer the PF amount from present account to the account valid after transfer to Madhya Pradesh Br.
Further PF has to be paid only by the branch where the employee has been transfered.

Thanks for your reply.
Form No. 13 is used to file by present employer for transfer of P.F. from one company to another company with in state of Maharashtra.
But In the Coal Mines Provident Fund Act there is different forms for transfer of P.F. like Form Trans 4 & 21.
Here, present employer is Madhya Pradesh Br. where the CMPF is applicable. Then which form I need to file to CMPF Commissioner, Form No. 13 or Form Trans 4?
