Dear Sir/Madam,

We would like to inform you that management has noticed that in the last three months, you have taken leaves (full day or short leave) mostly without any prior intimation to the company, and it seems to be a regular practice. Furthermore, please note that no half-day/full-day leave will be granted unless you inform the management at least three days before availing any leave.

Kindly treat this as a first warning.

Hi!

Please find the attached warning letter for not coming regularly. If the employee is not coming regularly, before giving him the warning letter, explain to him orally. If he continues the same, then plan for the warning letter. You can ask the concerned department heads or supervisor of that department to take care of that issue and can ask to levy a penalty.

Hi Gaurav,

I think you can prepare a letter by mentioning two things:

1) You are on uninformed leave from such and such a date.
2) If you don't report back to the concerned person within ---- hours, then your services will be terminated.

Mention these things, and similarly, if he doesn't return, then terminate him from the company. I hope this will solve your problem.

Dear (name),

Your work has been excellent. However, your poor attendance record is now overshadowing your work record. I realize that your health has been poor, but one of our job requirements is regular attendance. We have difficulty scheduling when we cannot rely on employees due to frequent absenteeism.

This subject has been discussed with you on several occasions. Now your attendance must improve to meet our requirements, or we will have to take further disciplinary action, possibly including termination.
