Hi, I hope this is not the first time you are revising the salary. In the past, salaries should have been revised on an annual basis, and an increment letter should have been issued. You can follow the same format. These are company-specific, and there is no standard template. You just congratulate and thank the employee for their performance during the period under review and then state that their salary stands revised from ___ to ___ with effect from ___.
