Salary slip is to be issued by your employer, the salary slip made by anyone else is meaningless and invalid. The gross salary has to be subdivided into its components, the deductions made as well as the net salary paid are to be indicated with respect to each month for which the salary was paid. There are template samples available in the net, you can prepare one if you need so.
