Again Employee rejoin in my organisation what are the joining formalities for that employee

This depends upon the re-employment policy of the company. If such policy exists, then you need to induct the previous employee of the company, afresh. You have to fulfil all joining formalities with him, as if a new employee only. Even his ID No also will have to be changed. Only his PF UAN No and ESI IP No (if he fall under PF/ESI) can be used /re-used. All other identifications shall have to be changed.

The rejoining of an employee is to treated as a new joining. Same formalities to be followed as you have been doing in other cases.
The employee to be issued with new Token No and his name to be bear in workman register and to mention the date he joined his work in your organisation.
