ESIC certificate of entitlement in Form 105 has to be filled by the employer when any employee works in another location where his registered ESIC dispensary is not available. Simply this form is used when employee is on temporary duty or authorized leave. Employee has to take ESIC Form 105 from his employer and it has to be submitted to the new ESIC dispensary.
This Form 105 is to be downloaded from IP portal.
