Online only we have to submit the accident report. Once you login to your ESIC Portal there left side top accident report (form 12) option will be there. Once you fill the report hard copy of the report you have to take and give it to employee. They needs to bring that along with company ID card and visit to esi office. IF you submit after 48 hours then you can't claim anything from ESIC. Do it ASAP.

Suppose one employee come to company and he has faced accident or evening going to home from the company in that case, is it mandatory to fill up accident form ?????
Because dispensary doctor always demand accident form
