Proposal Quotation Format for Civil Works Manpower

I am looking to find a good format for a proposal quotation to send to the principal contractor for the supply of civil works manpower to a construction site. We are a subcontractor firm that has just started our business to supply civil manpower to the principal contractor, who is directly working under the client company. The principal contractor will pay us on a work measurement basis. The rate of measurement will be as follows:
1. Bar Bending - 3800 rupees per metric ton.
2. Shuttering - 150 rupees per sq ft.

We want to send a quotation for the above rates with an increased price rate (4500 Rs. per MT and 200 Rs. per sq ft.) which they would review and agree to sign if accepted. Kindly provide me with the format for the same. It may contain:
